Youth and Family Services-Demo1

Summit Pointe provides a broad scope of quality services for youth and families. They range from
therapy-based programs for the individual child to non-therapeutic programs that engage the whole family.

Home-based

Home-based services are more flexible because the services occur in the home and community and can be scheduled more often than is usually true of outpatient counseling. Sometimes Home-Based services are recommended when a child and family have more intensive needs and services are focused on both the child and the family. The goals for Home-based services, how often sessions are scheduled, and what approaches will be used are decided together by the child, family, and Home-based worker. Home-based services are designed to support the child remaining in their family home. Home-based services may also be used when children and families who have been separated are working on being reunified.

 

Wraparound

Wraparound is a planning process used when children and families need a higher level of support to achieve their goals and when the child is at risk of being unable to stay at home or in the community. The Wraparound process starts with the facilitator bringing together the child, his or her family, and other people the family wants to involve – for example, a special education teacher or a foster care worker. The Wraparound team identifies the child and family’s strengths and needs and decides how to help them meet their goals. The Wraparound team meets at least once a month to ensure all parts of the plan are going well and adjust the plan if needed. Wraparound usually lasts several months to a year based on the progress in meeting the goals. When it is time to finish the Wraparound process, the team will figure out what service providers will continue to work with the family – for example, outpatient counseling, Community Living Supports (CLS), and psychotropic medication follow-up.

Intensive Crisis Stabilization

Intensive Crisis Stabilization (ICS) for youth provides quick, short-term help for kids and young adults aged 0-21 going through a serious emotional or behavioral crisis. When a crisis occurs, Summit Pointe sends trained professionals to the youth’s home, school, or another familiar place to
help calm the situation and avoid going to the hospital. A team from Summit Pointe evaluates the young person’s mental health and makes a personalized plan to reduce risks and bring emotional stability We offer short-term therapy and strategies to help the youth manage the crisis, while also involving the family in the process.

ICS focuses on keeping kids in safe, familiar environments while working through their crisis, helping reduce stress and speed up recovery. The Summit Pointe team works closely with the family or guardian(s), providing guidance and training on how to support the child during and after the crisis.

After the crisis is under control, the youth is connected to ongoing mental health services, like therapy or counseling, to help them stay healthy in the long run. This approach ensures a supportive and safe space for the youth while promoting long-term mental health and preventing future crises.

Case Management - Youth and Family Services

Case management helps families and youth find services that are goal-oriented and individualized. Services include assessment, planning, advocacy, coordination, and monitoring. This can include dental services, financial assistance, housing, employment, education, social services, and more as determined by need.

Peer Supports - Youth and Family Services

Peer Supports and Mentors offer encouragement and the benefits of their lived experience. They are people with either a mental illness or developmental disability who have learned successful life strategies through their own experiences.

Peer Supports and Mentors know that being a self-advocate starts with self-determination. They know the benefits of living independently. In addition, they understand how to be part of the community and navigate housing, education, benefits, transportation, and entitlements. Peer Supports and Peer Mentors involve friends and family as determined by the individuals they support.
